Health Coach Associate (ID: 699805)

Responsibilities:

• Conduct thorough health assessments, analyzing residents' health status and risk factors.
• Collaborate with residents and relevant stakeholders to establish health goals using the S.M.A.R.T. framework.
• Create customized health coaching plans that motivate residents towards achieving their wellness objectives.
• Assist residents in overcoming challenges related to lifestyle and behavioral changes.
• Monitor and document residents' progress and health outcomes to ensure effective care.

Requirements:

• Diploma or Advanced Diploma in a healthcare-related discipline.
• Strong ability to work independently as well as collaboratively within a team.
• Excellent communication and engagement skills, particularly with elderly.
• Demonstrated organizational, coordination, and interpersonal capabilities.
• Proficient with computer skills and Microsoft Office applications.
