Medical Social Worker

• Provide psychosocial assessment and support to patients and their families to address social, emotional and financial needs.

• Assess patients' social circumstances and identify appropriate support services and interventions.

• Coordinate with healthcare professionals and community agencies to facilitate holistic patient care.

• Provide counselling, case management and crisis intervention where appropriate.

• Assist patients and families with financial assistance, healthcare subsidies and relevant social support schemes.

• Facilitate discharge planning and referrals to community care and support services.

• Maintain accurate and confidential case records and documentation.

• Participate in multidisciplinary team discussions and other departmental programmes and initiatives

Requirements

• Degree or Graduate Diploma in Social Work

• Registered with the Singapore Association of Social Workers

• Passion to serve and provide good patient care

• Good interpersonal skills and able to work independently
