Pharmacist - Outpatient (Office Hours) - Central/up to $5k #HAO

Salary: up to $5000

Working Days & Hours: • Mon - Wed 8.30 to 5.15

• Thurs - Fri, 8.30 to 5.30

• One Saturday in every 4 weeks, 8.30 to 12.30pm

Location: Novena

Job Description:
* Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions

* Medication counselling on transfer medications by a variety of means (in-person and virtual)

* Medication reconciliation

* Review discharge prescriptions appropriately

* Verify new orders by the physicians and identify inappropriate medication/doses if relevant

* Assist with perform medication supply duty

* Follow up on medication preparation status

* Undertake duties which may be assigned by the staff member depending on the requirements

Job Requirements: Min Degree in Pharmacy & VALID AHPC w min 2 yrs of relevant inpatient ward experience
