Job Responsibilities

- Provide comprehensive nursing care to patients in their homes according to individualized care plans

- Perform nursing procedures, including medication administration, wound care, catheter care, and other clinical interventions

- Assess patients' conditions, monitor progress, and report any changes to the multidisciplinary team

- Educate patients and caregivers on disease management, medication administration, and home care techniques

- Maintain accurate clinical documentation and ensure continuity of care through effective communication with healthcare professionals

Requirements

- Valid SNB registration as a Staff Nurse

- Minimum 1 year of clinical nursing experience (home care experience is an advantage)

- Comfortable travelling to patients' homes across Singapore

- Strong communication, interpersonal, and time management skills
