Pharmacist / Senior Pharmacist (Outpatient Pharmacy) #HJO

Job Description

• Provide pharmaceutical care including dispensing and counselling of medications
• Educate patients and caregivers on proper medication use and adherence
• Identify, resolve, and prevent medication-related issues
• Ensure accurate and timely dispensing of medications
• Support safe administration of medications and provide guidance to healthcare staff
• Maintain proper documentation and comply with pharmacy standards and protocols
• Participate in quality improvement initiatives and projects
• Collaborate with healthcare teams to ensure optimal patient care
• Perform other duties as assigned

Requirements

• Degree in Pharmacy or equivalent, recognised by the Singapore Pharmacy Board for registration
• Strong communication and patient counselling skills
• Good attention to detail and accuracy
