About this job
- Working hours: Monday to Friday 9am-6pm
- Location: Tagore (Shuttle bus provided AMK mrt station)
- Salary : $2500- $3500
Responsibilities
- Coordinate and manage ICS referrals from hospitals
- Contact clients and families to arrange the start of care services
- Coordinate Nursing Aide/Caregiver assignments and schedules
- Monitor daily attendance and ongoing client services
- Arrange replacement staff when necessary
- Communicate regularly with clients, families, hospitals and the ICS team
- Monitor clients' service periods and upcoming discharge dates
- Assist on simple admin task
Requirement
- Diploma or relevant experience in Healthcare, Nursing, Community Care, Social Services or Healthcare Administration
- Comfortable coordinating staff schedules and handling last-minute changes
