About this job

Nursing Care Coordinator
Salary: $2500 - $3500
Working Hours: Monday - Friday | 9am - 6pm
Location: Tagore Lane (Shuttle bus provided AMK mrt station)

Responsibilities

Coordinate and manage ICS referrals from hospitals
Contact clients and families to arrange the start of care services
Coordinate Nursing Aide/Caregiver assignments and schedules
Monitor daily attendance and ongoing client services
Arrange replacement staff when necessary
Communicate regularly with clients, families, hospitals and the ICS team
Monitor clients' service periods and upcoming discharge dates
Assist on simple admin task

Requirement

Diploma or relevant experience in Healthcare, Nursing, Community Care, Social Services or Healthcare Administration
Comfortable coordinating staff schedules and handling last-minute changes
