Job Summary
The Social Worker / Social Work Associate supports individuals and families affected by blood cancers throughout diagnosis, treatment, recovery, and survivorship by providing psychosocial and practical assistance and connecting them to community resources.
Responsibilities
- Provide psychosocial and practical support tailored to blood cancer patients and caregivers to help them manage diagnosis, treatment, recovery, and survivorship challenges
- Conduct needs assessments to identify individual patient and caregiver support requirements and develop appropriate care plans
- Collaborate with hospital medical social workers, healthcare professionals, and community partners to ensure continuity of care and coordinated support
- Facilitate referrals to LLF programmes and relevant community services, and assist patients and caregivers in navigating psychosocial, financial, and community resources
- Follow up with referred patients to monitor ongoing support and adjust interventions as needed
- Support and facilitate patient and caregiver support groups, peer-support activities, and community programmes to enhance psychosocial wellbeing and survivorship
- Assist in developing and delivering patient education talks, workshops, and engagement activities to promote awareness and participation
- Encourage patient and caregiver involvement to build supportive peer networks and community connections
- Support LLF’s development of community-based survivorship services by helping patients navigate practical issues such as social reintegration, family adjustment, and return-to-work challenges through appropriate referrals
- Strengthen linkages between clinical care and community-based support by working closely with healthcare and community partners
- Maintain accurate, confidential case notes, referral records, and service documentation in compliance with LLF policies and data protection requirements
- Track patient engagement, follow-up activities, and programme outcomes to support service monitoring and evaluation
- Collect service data and feedback to contribute to programme evaluation and reporting efforts
- Build and maintain effective working relationships with hospitals, healthcare professionals, social service agencies, and community partners
- Participate in meetings, patient forums, and outreach activities to represent LLF and support patient services
- Support the Programme & Services team in developing new initiatives based on emerging patient and caregiver needs
- Perform other duties that support LLF’s patient services and programmes as required
Required competencies and certifications
- Degree in Social Work or recognised equivalent (for Social Worker role)
- Eligibility for accreditation with the Singapore Association of Social Workers (SASW), where applicable (for Social Worker role)
- Diploma or relevant qualification in social work, social services, psychology, counselling, community care, or related discipline (for Social Work Associate / Assistant role)
Preferred competencies and qualifications
- Experience in healthcare, oncology, palliative care, or community care (advantageous for Social Worker role)
- Relevant experience working with patients, caregivers, or vulnerable populations (advantageous for Social Work Associate / Assistant role)
- Fresh graduates with strong interest in oncology and community-based patient support may be considered (Social Worker role)
