Medical Social Worker

Responsibilities

• Conduct psychosocial assessments to identify patients’ social, emotional, financial and family-related needs.
• Provide counselling, emotional support and crisis intervention to patients and their families.
• Develop and implement appropriate social work intervention plans based on individual patient needs.
• Coordinate referrals and connect patients with community resources, financial assistance and relevant support services.
• Collaborate with multidisciplinary healthcare teams to support discharge planning, continuity of care and complex case management.
• Maintain accurate case records, prepare reports and ensure professional, ethical and confidential social work practice.
  

Requirement

• Degree in Social Work or related courses
• Able to commit to Monday to Friday office hours, occasional half Saturday
• Min 2 years or relevant experience
