Job Summary

Provide safe, person-centred nursing care to older persons, including those with dementia, by conducting clinical assessments, managing care plans, and supporting multidisciplinary teams in centre-based and home-care settings.

Responsibilities

• Conduct comprehensive clinical assessments to evaluate seniors’ health status and care needs
• Develop and implement individualised care plans to ensure effective, person-centred care delivery
• Manage medication administration and monitor patient responses to treatments
• Maintain accurate and timely documentation of nursing care and patient progress using digital care systems
• Respond promptly and appropriately to medical emergencies to ensure client safety
• Guide, coach, and support Care Executives and care staff in approved care practices, documentation, infection control, and client safety protocols
• Apply knowledge of infection prevention, risk management, and nursing standards to maintain a safe care environment
• Collaborate effectively with seniors, family members, and multidisciplinary care teams to optimise care outcomes
• Exercise sound clinical judgment independently within the Singapore Nursing Board scope of practice and professional competence
• Support quality management, audit processes, risk management, and continuous service improvement initiatives where applicable
• Provide care and support in both centre-based and home-care services as assigned
• Demonstrate compassion, patience, and commitment to delivering safe, person-centred care to older persons, including those living with dementia

Required competencies and certifications

• Diploma or Degree in Nursing from a recognised institution
• Registration with the Singapore Nursing Board (SNB) and possession of a valid Practising Certificate throughout employment
• Current Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED) certification, or willingness to obtain/renew within the company’s specified timeframe

Preferred competencies and qualifications

• Nursing experience in eldercare, dementia care, community care, day care, or home care
• Leadership, supervisory, or team-coordination experience for Senior Staff Nurse / Lead Nurse roles
• Experience or knowledge in quality management, audit, risk management, and continuous service improvement
• Comfort with using digital care documentation and administrative systems
