Perm Medical Social Worker (Govt Hospital) - #HKH

Responsible for delivery of patients’ service in the work area in a personalized, specialized and cost-effective way in accordance to established hospital policies, procedures and standard of practices.

Primary Responsibilities and Duties

• To assess for mental health issues, provide case formulation and recommendations for treatment.
• To provide appropriate counseling / psychotherapy for identified inpatients and outpatients.
• To contribute, as part of an inter-disciplinary team, in the overall care plan for the patient.
• As part of an inter-disciplinary team, to facilitate transition to adult-care.
• To develop or improve on workflow for better patient care.
• To initiate and implement new psycho-social programmes
• To assist in the development of materials for teaching and training.
• Provide in-service training for department staff as needed.

Requirements:

• Min. Degree in Social Work
• No exp needed - training provided
• Min 2 to 4 years of social worker experience will be an advantage
