Home Care Staff Nurse

Roles & Responsibilities

• Provide home-based nursing care services requiring substantial and specialised nursing skills
• Conduct caregivers' training onsite at the client's place of residence
• Anticipate needs and potential risks and provide advice to mitigate them
• Plan, develop, deliver and evaluate person-centred nursing care plans with consideration of clients' needs and preferences in a timely manner
• Conduct initial assessments and 6-monthly reviews in a timely manner
• Execute care plans and care interventions appropriately and in a timely manner
• Practice infection control measures per organisational policy
• Manage patient workload through efficient planning and timely scheduling based on client needs
• Complete timely and accurate documentation after home visits within 1 working day
• Comply with service guidelines and policies for safe and appropriate care
• Participate actively in homecare multidisciplinary meetings and team huddles

Job Requirements

• Registered with the Singapore Nursing Board with a valid practising certificate
• Minimum 4 years of ongoing nursing experience in a Medical or Geriatric setting (acute or community), within the past 2 years
• Experience in Oncology or Palliative Care is an advantage
• Good communication and interpersonal skills, comfortable interacting with people from all walks of life
• Good attitude towards learning and receiving feedback
• Adaptable and collaborative team player, responsive across team communication channels
• Independent, highly reliable, and consistent in performance
• Sound clinical judgement in a home-care setting
• Keen interest in supporting End-of-Life (EOL) care for homebound seniors
• Willing and able to support After-Office-Hours (AOH) care for EOL patients on a rotational basis (tele-consult)
• Physically able to carry work equipment and travel within the community
