Nurse Manager (Community Hospital) #HJO

Job Responsibilities

• Lead and manage the nursing team to ensure safe, effective and patient-centred care within the assigned ward or service.
• Oversee day-to-day ward operations, nursing manpower deployment, rostering and allocation of resources.
• Ensure delivery of high-quality nursing care for patients requiring sub-acute, rehabilitative and continuing care.
• Monitor patients’ clinical outcomes and ensure appropriate escalation and management of changes in patients’ conditions.
• Work closely with doctors, allied health professionals, medical social workers and other care teams to facilitate multidisciplinary care planning.
• Coordinate discharge planning and continuity of care with patients, caregivers and community care partners.
• Lead quality improvement initiatives, audits and reviews to improve nursing practices and patient outcomes.
• Conduct staff performance reviews, identify training needs and support succession and workforce development.
• Participate in hospital-wide nursing committees, projects and service development initiatives.
• Support nursing education, clinical training and professional development activities.
• Perform other duties as assigned by Nursing leadership.

Requirements

• Degree in Nursing or equivalent recognised nursing qualification.
• Valid registration with the Singapore Nursing Board (SNB) as a Registered Nurse.
• Significant post-registration nursing experience with demonstrated experience in a nursing leadership / supervisory capacity.
• Prior experience in a Community Hospital, rehabilitation, geriatric, sub-acute or inpatient care environment will be highly advantageous.
• Strong clinical knowledge with the ability to oversee the care of patients with complex medical, rehabilitation and psychosocial needs.
• Strong leadership, coaching and people management capabilities.
