*URGENT* Palliative Nurse - 5 Days Office Hours, up to $5K++ #HXY

• Provide safe, competent, and individualised nursing care

• Assess, monitor, and evaluate patients’ conditions, develop appropriate care plans, and support patients and families in managing advanced illnesses.

• Coordinate patient care with physicians and multidisciplinary teams

• Build strong relationships with patients and caregivers, assess home care needs, and recommend appropriate training, equipment, and support services.

• Ensure continuity of care by following up on treatment plans and coordinating necessary resources.

• Perform nursing procedures including medication administration, wound care, and other clinical interventions as required.

Requirements:

• Minimum Diploma in Nursing or above

• Advanced Diploma in Palliative Care Nursing (ADPCN) or the Specialist Diploma in Palliative Care Nursing (SDPCN) is required

• Able to commit 5 days office hours

• Able to start within short notice
