Medical Social Worker/Senior Medical Social Worker

A leading healthcare and eldercare organisation is seeking Medical Social Workers / Senior Medical Social Workers to join its growing nursing home team. The role focuses on providing psychosocial support, case management and care coordination to support seniors and their families throughout their care journey.

Job Responsibilities:

• Conduct psychosocial and financial assessments for residents and develop person-centred care plans
• Provide counselling, emotional support and psychosocial interventions to residents and their families
• Collaborate with multidisciplinary teams including nursing, therapy and healthcare professionals to support residents’ overall well-being
• Coordinate referrals and work with community partners, healthcare providers and relevant stakeholders
• Support residents and families in accessing financial assistance schemes and community resources
• Facilitate family discussions, care planning conversations and caregiver support when required
• Maintain accurate case documentation, reports and resident records
• Participate in quality improvement projects and initiatives to enhance resident care
• Provide guidance and mentorship to junior staff, interns and support team members (for Senior Medical Social Worker level)

Job Requirements:

• Min. Postgraduate Diploma in Social Work recognised by SASW
• Registered Social Worker (RSW) with the Singapore Association of Social Workers (SASW)
• Min. 2 – 5 years years of relevant social work experience
• Experience in healthcare, nursing home, intermediate or long-term care settings will be an advantage
• Supervisory or mentoring experience will be an advantage for Senior Medical Social Worker level
• Strong counselling, case management and communication skills
• Able to work independently and collaboratively within a multidisciplinary team
• Compassionate, organised and resident-centred approach
• Proficient in Google Workspace (Gmail, Docs, Sheets and Slides)
