Social Worker

Job Summary

The Social Worker delivers casework management, counselling, and social intervention to individuals and families facing complex psychosocial challenges, focusing on assessments, personalized care plans, and connecting clients with community resources for sustainable self-reliance.

Responsibilities

• Conduct comprehensive intake and biopsychosocial assessments to evaluate client needs and challenges.
• Design, implement, and monitor individualized intervention and care plans aligned with client goals.
• Provide evidence-based casework management and counselling to support client well-being.
• Coordinate with multiple agencies to link clients to financial, medical, educational, and community resources.
• Perform regular home visits, client reviews, and crisis assessments to ensure safety and progress.
• Maintain accurate, timely, and compliant case documentation and reports.
• Participate actively in multidisciplinary case conferences and professional supervision to enhance service delivery.

Required competencies and certifications

• Bachelor’s Degree or Graduate Diploma in Social Work from a recognized tertiary institution.
• Registered Social Worker (RSW) or Accredited Social Worker (ASW) status with the Social Work Accreditation and Advisory Council (SWAAC) preferred, or eligibility for accreditation.

Preferred competencies and qualifications

• Proficiency in casework management, risk assessment, and crisis intervention techniques.
• Strong active listening, empathetic communication, and problem-solving skills applied in client interactions.
• Ability to work independently and collaboratively within multidisciplinary teams.
• Basic competence in using case management software and digital tools.
