Responsibilities:

Outreach to Community

  • Support outreach events to community to promote mental wellness and increase public awareness of mental health and sharing of community support mechanisms and resources available

Basic Recognition of Signs and Symptoms of Mental Health Conditions

  • Identify persons who are 18 years old and above and at-risk / diagnosed with mental health conditions
  • Connecting them with right service for early diagnosis and treatment
  • Conduct mental health screening for clients after community outreaches, referred from their families, grassroots organizations / civic groups and other service partners as well as those at risk within the community with an agreed upon identification / assessment tools

Basic Emotional Support, Service Linkages and Follow Up

  • Provide basic emotional support and monitoring to clients or caregivers of persons with dementia or mental health conditions through regular home visitations
  • Support the implementation of casework interventions
  • Support crisis management in accordance to organisation’s policies and procedures

Network and Engagement of Community Partners

  • Support engagement with key community partners such as grassroots organisations, social service agencies and religious organisations to raise their mental health awareness, understand services provided by CREST and coordinate mental health training and education

Administration and Documentation

  • Submit reports to external parties as specified
  • Ensure proper maintenance of case documentations
  • Provide administration and logistical support for programmes

Other Duties

  • Attend all internal and external meetings
  • Any other duties as assigned

Requirements:

  • At least a Diploma in Social Work, Counselling, Psychology or other fields related to healthcare or community care
  • Preferably 2 to 3 years of healthcare/community care experience
  • Passionate to work in the mental health sector and community services
  • Good communication skills (oral, written, bilingual) and interpersonal skills
  • A team player who can also work independently
  • Detail-oriented in keeping proper documentation
  • This position is on a contract basis
