Senior/Social Worker

Responsibilities:

• Conduct casework and deliver psychosocial support to patients, caregivers, and families.
• Review intervention plans and collaborate with all relevant stakeholders, including external agencies.
• Manage patient admissions and perform assessments for financial assistance.
• Plan, organise, and oversee social programmes tailored for patients.
• Maintain and update patient records and billing information.
• Lead and facilitate group work and programmes designed for targeted client groups.

Requirements:

• Degree in social work with accreditation.
• Minimum of 2 to 3 years of relevant experience.
• Excellent teamwork abilities, along with effective written communication and interpersonal skills.
