Social Work Coordinator / Care Coordinator

Key Responsibilities:

• Conduct pre-admission screenings and develop individualized care plans for patients and caregivers.

• Coordinate with nurses and clinicians to provide holistic home care, monitor patient progress, and report any care gaps or changes in condition.

• Manage patient and caregiver enquiries, provide care plan updates, and maintain accurate documentation of care and treatment progress.

• Participate in interdisciplinary team meetings and collaborate with community resources to ensure continuity of care and appropriate referrals.

• Monitor patients' vital signs through telehealth systems, escalate abnormalities promptly, and ensure compliance with telehealth monitoring protocols.

• Deliver basic patient education and health coaching on lifestyle changes, including diet, nutrition, and exercise.

• Support operational excellence by coordinating patient care equipment and supplies, participating in quality improvement and risk management initiatives, and carrying out other assigned duties as needed.
  

Requirements:

• Diploma in Social Work / Health Services Management or equivalent

• Strong interpersonal, communication, and counseling skills

• Empathy, patience, and the ability to work with people from diverse backgrounds

• Familiarity with local community resources and social services
