Key responsibilities:

• Provide client-centred physiotherapy in home and community settings to improve mobility, independence and quality of life.
• Conduct assessments of clients’ physical, functional and mobility needs.
• Develop and implement individualised treatment plans based on clinical findings and client goals.
• Provide evidence-based therapy to improve strength, balance, endurance and mobility.
• Educate clients and caregivers on exercises, fall prevention and safe mobility techniques.
• Maintain accurate and timely clinical documentation.
• Work closely with multidisciplinary teams to provide holistic care.
• Participate in case conferences, community outreach and health promotion programmes.
• Supervise therapy assistants and support clinical student placements.
• Participate in internal training and provide physiotherapy services across different settings, such as Home Rehab, Community Rehabilitation, Community Hospital and Nursing Homes, based on competency.

Qualifications and Skilled Required

• Diploma/Degree in Physiotherapy
• Fully Registered with the Allied Health Professions Council in Singapore
