ROLES AND RESPONSIBILITIES

  • Provide bio-psychosocial assessment and intervention to support patients / residents / clients / caregivers in accordance with professional standards of practice
  • Deliver end-to-end social work services, including case assessment, care planning, intervention, evaluation, counselling, and emotional support
  • Assist in addressing psychosocial, financial, and discharge planning needs as part of overall care management
  • Collaborate closely with the multi-disciplinary team (e.g. doctors, nurses, therapists) to ensure holistic, patient-centred care
  • Identify service gaps and emerging needs; develop, coordinate, and facilitate group work programmes to strengthen social support systems, build community networks, and enhance coping and resilience among target groups
  • Conduct outreach to individuals and groups who may benefit from group interventions
  • Provide guidance, training, and co-supervision to social work undergraduates on fieldwork placement and newly qualified Medical Social Workers (MSWs)
  • Support development of competencies and adherence to professional standards
  • Participate in departmental and organisational projects, including quality improvement initiatives, to enhance service delivery and patient outcomes
  • Remain vigilant in identifying potential risks associated with social work practice and patient care
  • Support organisational risk management efforts by taking appropriate actions to mitigate risks and ensure safe, effective service delivery in alignment with Ren Ci Hospital’s policies

JOB REQUIREMENTS

  • Degree in Social Work (or major in Social Work) from a recognised institution
  • Accredited or eligible for Practicing Social Worker (SASW) accreditation
  • 2–3 years of relevant experience in social work, preferably in healthcare or community settings
  • Experience in case management, including psychosocial assessment, intervention, and care planning
  • Exposure to working with patients / clients / caregivers with diverse and complex needs
  • Experience in multidisciplinary settings and group work or community outreach is advantageous
  • Strong social casework and intervention skills
  • Good counselling and emotional support skills
  • Effective communication and interpersonal skills, with ability to engage diverse stakeholders
  • Ability to work collaboratively within a multi-disciplinary team
  • Demonstrates self-awareness, empathy, and ethical practice in working with vulnerable individuals
  • Good organisational and time management skills, with ability to manage multiple cases
  • Proficient in Microsoft Office (Word, Excel)
  • Able to identify and respond to practice risks and contribute to effective risk management
  • Independent, adaptable, and committed to continuous learning and professional development
