Occupational Therapist Locum (Home Therapy)

JOB RESPONSIBILITIES

• Conduct occupational therapy assessments and develop individualized treatment plans to improve patients' functional independence and quality of life.
• Perform home environment assessments to identify safety, accessibility, and fall risks.
• Recommend and prescribe assistive devices, adaptive equipment, and home modifications to support ageing-in-place.
• Evaluate treatment outcomes and modify interventions according to patients' changing needs.
• Collaborate with multidisciplinary teams, patients, caregivers, and external agencies to ensure coordinated care and discharge planning.
• Educate, train, and support patients and caregivers in managing daily living activities safely and effectively.
• Maintain accurate, timely, and confidential clinical documentation and patient records.
• Supervise therapy aides and support the supervision of occupational therapy students where required.
• Participate in professional development, departmental meetings, training, quality improvement initiatives, and research-related activities.
• Liaise with community partners, voluntary welfare organisations, and relevant agencies to facilitate continuity of care.
• Comply with professional, organisational, infection control, health and safety, and risk management requirements.
• Identify, report, and mitigate potential risks, hazards, and incidents in accordance with organisational policies.
• Perform any other duties and projects assigned by Management.

JOB REQUIREMENTS

• Degree in Occupational Therapy recognised by MOH and the Allied Health Professions Council (AHPC).
• Registered with AHPC and holds a valid Practising Certificate.
• Postgraduate qualification or relevant certification is an advantage.
• Experience in community, rehabilitation, or home care settings is preferred.
• Strong clinical assessment, treatment planning, and problem-solving skills.
• Ability to work independently in patients' homes and community-based settings with sound clinical judgement.
• Good communication, interpersonal, and multidisciplinary team collaboration skills.
• Proficient in clinical documentation and report writing.
• Compassionate, patient-centred, empathetic, and adaptable.
• Strong organisational and time management skills.
• Committed to continuous professional development, quality improvement, and patient safety.
• Willingness and flexibility to travel to patients' homes and community settings as required.

WORK LOCATION

Primary Work Location: Ren Ci Woodlands Nursing home (17 Woodlands Dr 17, Singapore)
Home Therapy Coverage Area: North, Northwest, and West regions of Singapore

WORK ARRANGEMENT

• 2 workdays per week.
• Travel to patients' residences and community-based settings as required for assessment, treatment, caregiver training, and follow-up interventions.
• Maintain flexibility to support service delivery across assigned geographical zones based on operational needs.
