Key Responsibilities
- Manage walk-in, online and telephone enquiries relating to healthcare and home care services.
- Assess clients' needs using appropriate screening tools and recommend suitable services or care packages.
- Coordinate with internal teams and external healthcare and social service partners on service referrals.
- Submit referral documentation and follow up on outstanding action items to ensure timely service delivery.
- Coordinate and administer services requested by clients.
- Ensure timely and accurate documentation in relevant systems and physical records.
- Work closely with internal stakeholders to ensure the smooth and effective delivery of Home Care services.
- Assist with appointment scheduling and coordination for various home care services.
- Conduct regular monthly home visits and telephone follow-ups to engage clients and caregivers, build rapport and gather feedback on services provided.
- Collaborate with healthcare and social service partners to recommend appropriate care programmes based on clients' needs.
- Assist with appeals and applications for relevant financial or social support when required.
- Counsel and educate caregivers on appropriate care practices, available support services and financial assistance.
- Conduct six-monthly reassessments of care plans to determine clients' ongoing service needs.
- Work closely with the Operations and Nurse Manager to facilitate the appropriate discharge of clients from services.
- Participate in programme development, review and evaluation initiatives to address the evolving needs of the eldercare sector.
- Provide feedback and contribute ideas to enhance service quality, operational processes and client outcomes.
- Support initiatives to drive revenue growth and enhance service utilisation.
- Identify opportunities, optimise available resources and implement strategies to achieve business KPIs.
- Contribute to improving client satisfaction, service utilisation and overall operational performance within the assigned zone(s).
- Support other ad-hoc duties as assigned by supervisor.
Requirements
- Min. Diploma in any related field.
- Minimum 3 years of relevant experience in community healthcare settings in Singapore.
- Proficient in Google Workspace and Microsoft Office applications.
- Prior experience in customer service is an advantage.
