Nurse Case Manager / Interim Caregiver Manager (Registered Nurse)

Agape Plus is seeking a Registered Nurse to serve as a Nurse Case Manager / Interim Caregiver Manager, providing clinical oversight, case management and supervision of caregivers delivering home-based care.

Key Responsibilities

• Assess clients' health conditions, functional needs, home environment and caregiving requirements.
• Review and develop individualised care plans and ensure care is delivered safely according to the prescribed care plan.
• Manage assigned client cases, conduct home visits and follow up with clients and families.
• Provide clinical oversight and supervision to caregivers and healthcare support staff.
• Monitor clients for changes in condition and ensure timely escalation to appropriate healthcare professionals or emergency services where required.
• Conduct structured client reviews, including midpoint reviews where applicable, and identify ongoing or longer-term care needs.
• Coordinate with operations teams on caregiver matching, deployment, replacements, scheduling and care handovers.
• Communicate with clients, families, hospitals, healthcare professionals and community care partners to support continuity of care.
• Participate in caregiver interviews, competency assessments, onboarding, coaching and training.
• Respond to incidents, complaints and care-related concerns and ensure appropriate documentation and follow-up.
• Maintain accurate care plans, assessment records, case notes, home-visit records and incident reports.
• Support quality assurance, infection prevention, audits, service reviews and continuous improvement.
• Ensure client confidentiality and compliance with applicable data protection and care requirements.
• Participate in an agreed escalation/on-call arrangement for urgent care matters when required.

Requirements

• Diploma or Degree in Nursing from a recognised institution.
• Registered Nurse with the Singapore Nursing Board (SNB) with a valid Practising Certificate.
• Preferably at least 2 years of clinical nursing experience.
• Experience in home care, community nursing, geriatric care, rehabilitation, palliative care or case management is advantageous.
• Able to assess care needs, develop care plans and recognise changes in clients' conditions requiring escalation.
• Experience supervising, coaching or training caregivers or healthcare support staff is preferred.
• Strong communication, interpersonal and case-management skills.
• Good documentation and report-writing skills.
• Comfortable using digital care-management systems and basic office applications.
• Willing to travel across Singapore for client home visits.
