Nurse Case Manager

Job Scope

1. Assess clients’ health conditions, care needs, home environment and support requirements.
2. Develop and review individualised care plans based on clients’ needs and changes in their condition.
3. Manage assigned cases, conduct home visits and follow up with clients and families.
4. Supervise caregivers, provide clinical guidance and ensure care is delivered safely and according to care plans.
5. Coordinate with the operations team on caregiver matching, deployment, replacements and care handovers.
6. Communicate with clients, families and healthcare professionals to address concerns and maintain continuity of care.
7. Respond to incidents and changes in clients’ conditions, ensuring appropriate escalation and follow-up.
8. Participate in caregiver interviews, competency assessments, onboarding and training.
9. Maintain accurate care plans, case notes, assessment records and incident reports while protecting client confidentiality.
10. Support care quality reviews, audits, infection prevention practices and service completion arrangements.

Job Requirements

1. Diploma or Degree in Nursing from a recognised institution.
2. Registered Nurse with the Singapore Nursing Board, with a valid Practising Certificate.
3. Preferably at least two years of clinical nursing experience. Experience in home care, community nursing, geriatric care or case management is an advantage.
4. Able to assess care needs, develop care plans and recognise changes in clients’ conditions requiring escalation.
5. Experience supervising or training caregivers or healthcare support staff is preferred.
6. Strong communication and interpersonal skills, with the ability to work effectively with clients, families and care teams.
7. Organised and able to manage multiple cases, prioritise tasks and maintain accurate documentation.
8. Comfortable using digital care management systems and basic office software.
9. Compassionate, patient and professional, with a commitment to client safety and confidentiality.
10. Willing to travel across Singapore for home visits and provide support for urgent care matters when required.
