About this job
This role is responsible for providing comprehensive care coordination and health management support to residents, ensuring that their healthcare, wellness, and overall wellbeing needs are addressed in a timely, seamless, and resident-centred manner. The incumbent will serve as a key point of contact between residents, families, healthcare providers, and internal teams, facilitating effective communication, care planning, medical navigation, and service delivery. Through strong coordination, advocacy, documentation, and follow-up, the role supports a safe, responsive, and high-quality living environment for residents.
Responsibilities
Care Coordination & Health Management
- Develop, implement, and review individualized care plans to support residents' healthcare and wellbeing needs
- Monitor residents' health status, vital signs, and care outcomes, ensuring timely updates and escalation of concerns
- Coordinate healthcare, rehabilitation, and wellness services to deliver seamless, resident-centred care
- Participate in multidisciplinary care planning and case reviews
Medical & Healthcare Navigation
- Coordinate medical appointments, specialist consultations, hospital visits, investigations, and follow-up care
- Arrange medical chaperone and transportation services where required
- Ensure timely communication and follow-up of medical recommendations, referrals, and treatment plans
Resident & Family Engagement
- Serve as the primary liaison for residents and their families.
- Provide regular updates on residents' health status, care plans, and significant developments
- Advocate for residents' needs, preferences, and overall wellbeing
Resident Experience & Wellness
- Support resident admissions, onboarding, and care transitions.
- Facilitate health screenings, wellness programmes, educational initiatives, and preventive health activities
- Manage resident feedback and continuously enhance service quality and resident experience
Partnership, Compliance & Operations
- Build and maintain effective partnerships with healthcare providers and hospitals
- Maintain accurate and confidential resident records and documentation
- Ensure compliance with regulatory, operational, and quality standards
- Support audits, incident management, emergency response coordination, and quality improvement initiatives
- Prepare reports, care summaries, and management updates as required
- Any other duties as assigned
Requirements
- Degree in Nursing with at least 3 years of relevant care coordination experience.
- Compassionate, patient and service-oriented.
- Strong communication, interpersonal and stakeholder management skills.
- Highly organised, detail-oriented, and able to effectively manage multiple priorities concurrently.
- Proactive with sound judgement and problem-solving abilities.
- Ability to engage and communicate effectively with elderly clients from diverse backgrounds, including those who are more comfortable conversing in Mandarin.
- Prior experience in senior living and healthcare with hospitality related fields will be an added advantage.
