Programme Executive, SD (4 months contract) - hybrid arrangement

• Conduct case assessment and formulate Individualized Care Plan for persons with dementia and their carers.
• Plan and facilitate group sessions/ activities for carers and/or persons with dementia.
• Provide intervention such as psychoeducation, supportive counselling and service linkages.
• Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
• Provide timely and accurate case recordings and relevant documentations.
• Participate in team meetings and case conferences as required.
• Assist promptly in the preparation of data and statistics for programme evaluation and reporting purposes.
• Explore new avenues of collaboration with community partners and stakeholders.
• Support in outreach events and talks.
• Any ad-hoc tasks as and when required.

Requirements

• Degree in Social work and Psychology, or any relevant discipline, with a minimum of 2 years of experience in community engagement and/or case management.
• Ability to conduct Advance Care Planning conversation or prior experience working with persons with dementia or carers will be an added advantage.
• Requires work on scheduled Saturdays and occasional weekday evenings.
• Good oral, written and interpersonal communication skills.
• Ability to facilitate group sessions will be an added advantage.
• Self-motivated with a high level of initiative.
• Resourceful and adaptable.
• Strong attention to detail.
• Good organizing and execution skills.
