Job Scope:
- Assess patients' lifestyle, dietary habits and diabetes management needs
- Provide diabetes education and personalised counselling
- Educate patients on insulin injections and glucose monitoring
- Review glucose profiles and support individualised care plans
- Work closely with Endocrinologists and support quality improvement initiatives
Requirements:
- Valid SNB
- Advanced Certification in Diabetes required
- Relevant diabetes nursing / patient education experience
