Cardiac Care Coordinator (Non Clinical Nursing)

Our esteemed client is seeking a Care Coordinator to support the Cardiac Department in coordinating patient care.
In this role, you will work closely with cardiologists, nurses, allied health professionals, primary care providers and community care partners to ensure patients receive coordinated and continuous care. You will be involved in patient assessment, care planning, treatment follow-up, patient education and monitoring of clinical outcomes.
Job Responsibilities:
Patient Assessment & Care Coordination
Identify and assess patients for suitability and eligibility for enrolment into the Transitional Shared Care Programme (TSCP).
Review patients’ suitability for enrolment with the clinician-in-charge and facilitate programme enrolment.
Develop and coordinate individualized follow-up plans according to patients’ clinical needs.
Monitor patients’ treatment progress, attendance and compliance with clinical protocols.
Coordinate and arrange follow-up appointments across cardiology clinics, polyclinics, Primary Care Networks and General Practitioners.
Facilitate smooth transitions of care between specialist and primary care settings.
Follow up with patients who default on appointments and implement appropriate interventions to improve compliance.
Liaise closely with cardiologists, nurses, allied health professionals and primary care partners to ensure continuity of care.
Track patients’ achievement of treatment targets and clinical outcomes.
Maintain accurate patient records and databases for case management, outcome monitoring and service evaluation.
Programme Monitoring & Database Management
Develop and maintain a database of patients enrolled in the programme.
Record and monitor patients’ attendance at primary care settings and cardiology clinics.
Track and document patients’ progress and compliance with the clinical protocol.
Record and monitor the achievement of treatment targets for enrolled patients.
Clinical Audit & Data Coordination
Collect acute myocardial infarction (AMI) mortality data and ensure timely submission to the Ministry of Health (MOH).
Collect relevant clinical information, including diagnostic variables, procedures, medications, intra- and post-procedure events, comorbidities, laboratory results and treatment plans.
Prepare reports and ensure timely submission to the Head of Department and relevant Clinical Governance Office.
Prepare and submit audited data accurately and within the required timelines to the relevant authorities.
Support clinical audit, quality improvement and service evaluation activities.
Perform additional operational and administrative duties as assigned by the supervisor.
Job Requirements:
Bachelor Degree in Science or Nursing.
Minimum 3 years of working experience in a healthcare setting.
Good verbal and written communication skills.
Strong interpersonal and communication skills, with the ability to engage and collaborate effectively with staff at all levels.
Positive, proactive and patient-focused mindset.
Good team player who is also able to work independently.
