6 Months Nurse Counsellor - HSN

We are hiring under Recruit Express headcount and you will be deployed to:

Location: Central

Working hour: 5 days (office hour)

Salary: $29/hour

Job Responsibilities

• Provide patient care and chronic disease counselling, including developing care plans, educating patients on lifestyle changes and medication adherence, and conducting diabetic foot screening and spirometry.

• Coordinate with GPs and Primary Care Coordinators for patient referrals, care coordination, follow-up, and clinical documentation.

• Support administrative and quality improvement projects, including clinical supervision and development of healthcare workflows and policies.

Requirements:

• Minimum Diploma in Nursing.

• Registered with the Singapore Nursing Board (SNB).

• Relevant nursing experience preferred.

• Good communication and interpersonal skills.
