Role & Responsibilities
• Conduct psychosocial and financial assessments for patients/clients and their caregivers
• Provide direct counselling, emotional support, and psychoeducation to patients/clients and their families to facilitate coping with illness, grief, loss, and life transitions.
• Collaborate with the multi-disciplinary team (MDT) to develop individualized care plans and facilitate family conferences to support communication and shared decision-making.
• Act as an advocate and liaison for patients/clients and their families, liaising with internal and external stakeholders, community partners, and government agencies to ensure access to necessary resources and services.
• Proactively risks to patient/client well-being and intervene promptly and appropriately to mitigate potential harm.
• Maintain accurate, comprehensive, and timely documentation of assessments, interventions, care plans, and progress notes in accordance with professional standards and organizational policies.
• Actively participate in and contribute to departmental and organizational quality improvement initiatives, service development, and program evaluation to enhance patient/client outcomes and service delivery.
• Uphold the highest standards of professional social work ethics, confidentiality, and practice as guided by the Singapore Association of Social Workers (SASW) Code of Ethics.
Qualifications, Competencies & Experience
• Master/Bachelor/Post Graduate Diploma in Social Work
• Preferably a registered Social Worker with the Singapore Association of Social Workers (SASW)
• Good team player with positive attitude, caring and compassionate individual
• Good interpersonal skills, able to communicate people at all levels such as patient’s families, government agencies and other professionals
• A quality service mind-set
• At least 3 years of relevant working experience in a healthcare or eldercare setting
• Familiarity with dialects will be an advantage
• Advance Care Planning facilitator would be an advantage
• Inclination towards palliative care and/or community case management will be an advantage
• Ability to speak and write effectively in English.
