Social Worker

HCSA Highpoint Halfway House is seeking a dedicated Social Worker to support the rehabilitation and reintegration of individuals under the Singapore Prison Service's Community-Based Programme.

As part of our Clinical Care team, you will provide case management, counselling, assessments, and support services to residents and their families while working closely with internal teams and external partners to facilitate positive outcomes.

Key Responsibilities

• Conduct assessments and develop intervention plans.
• Provide counselling and case management services.
• Support residents and families throughout the rehabilitation journey.
• Facilitate individual and group-based programmes.
• Collaborate with community partners and multidisciplinary teams.
• Maintain professional documentation and reports.
• Participate in programme development and quality improvement initiatives.

Requirements

• Degree or Postgraduate Diploma in Social Work from a recognised institution.
• At least 2 years of experience in social work, counselling, or case management.
• Strong communication, interpersonal, and stakeholder management skills.
• Experience in rehabilitation, addiction recovery, mental health, family services, or community work will be an advantage.
• Ability to work independently and within a multidisciplinary team.
