Youth Executive
About this job
1. PRIMARY RESPONSIBILITIES
1) Engage and conduct outreach to youth addressing youth issues such as addictions, substance abuse and bullying through schools, communities and churches.
2) Plan and implement preventive education programmes (such as workshops and talks) focused on drug awareness, addiction prevention, life skills, building resilience and sports.
3) Collaborate with sports agencies and like-minded organisations to organise sport activities, such as football, badminton etc to reach out to youth.
4) Provide case management and emotional support to the youth, working closely with their families, schools and other support systems.
5) Monitor the youth progress and prepare case reports and documentation.
6) Monitor and evaluate program outcomes and impact.
7) Assist in marketing, fundraising and publicity efforts, including social media content creation.
8) Manage and train a team of volunteers.
9) Manage administrative tasks such as budgeting, reporting and logistics coordination.
Case Management
1) Assess residents’ needs (e.g. accommodation and employment needs) and formulate Individual Service Plans (ISPs)).
2) Work closely with social worker to plan for interventions and make proper referral links to other services to meet residents’ needs.

Attend
to and manage residents’ day-to-day requests during consultation hours
(Mon-Fri, 2pm-4:30pm).
4) Upkeep residents’ documents, case/counselling notes and individual case files.
5) Monitor resident’s progress and provide updates for the monthly reports.
6) Track and evaluate the resident’s progress during their stay at the Halfway House. This includes involvement in case discussions among the teams and case conferences with Singapore Prison Service Officers.
Administrative & Audit
1) Assist social workers in preparation of report for monthly meeting and monthly halfway house reports for monthly submission to SPS.
2) Assist in preparation for half yearly HSM Audit by ensuring required documentation such as programs attendance, emplacement documents, case/counselling notes, employment documents, family contact logs etc., are in order.
