About The Role
The Straits Geriatrics Centre is looking for a Registered Nurse to join our growing geriatric care team. This is an opportunity for nurses who are looking to move beyond traditional hospital based nursing and develop their clinical skills in a geriatric, clinic and community care setting. You will work closely with our geriatrician and multidisciplinary team to provide holistic care for older adults, both in the clinic and in their homes.
Key Responsibilities
- Conduct nursing assessments and Comprehensive Geriatric Assessments (CGA) for elderly patients.
- Provide nursing care, procedures and treatments in both clinic and home-care settings.
- Perform nursing procedures including venepuncture and blood taking, injections and vaccinations, wound care, intermittent and indwelling catheterisation, health screening, ECG monitoring and interpretation, and other routine procedures.
- Assist doctors with medical procedures, support clinical interventions, and assist in the medicine dispensary.
- Support the management of patients with dementia, frailty, falls, osteoporosis and chronic medical conditions.
- Conduct home visits as part of the geriatric care team.
- Coordinate and schedule patient registrations, assist with payment collection, and ensure accurate maintenance of medical records.
- Provide patient and caregiver education on health management, medications, lifestyle, care plans and disease management.
- Coordinate patient care with doctors, caregivers and other healthcare professionals.
- Maintain accurate and timely clinical documentation - patient progress, treatment plans, medication records and charts.
- Ensure smooth day-to-day running of the clinic, maintaining operational efficiency.
- Participate in the development and improvement of geriatric and community nursing services, and other clinical or operational initiatives.
Requirements
- Diploma or Degree in Nursing.
- Registered Nurse with valid Singapore Nursing Board (SNB) registration.
- Minimum 3–5 years of clinical experience; prior experience in geriatrics, palliative care, community nursing or home care is highly preferred.
- Able to communicate well with elderly patients; dialect-speaking ability is a plus.
- Organised, responsible and attentive to detail.
- Compassionate and patient-centred, with a genuine interest in caring for older adults and a strong commitment to patient safety and clinical excellence.
