About this job

What you'll be doing

  • Lead the end-to-end implementation of community healthcare programmes, from planning and stakeholder coordination through to execution and evaluation

  • Work closely with public hospitals, community care partners, primary care providers and social service agencies to coordinate programme delivery

  • Translate programme objectives into practical operational plans, workflows, timelines and implementation activities

  • Coordinate community outreach, engagement activities, events and programme operations to ensure smooth delivery

  • Manage day-to-day programme requirements including resources, vendors, logistics, budgets and operational matters

  • Track programme participation, service utilisation and key performance indicators to monitor progress and outcomes

  • Gather feedback from healthcare and community stakeholders and identify opportunities to improve programme delivery and patient/community experience

  • Support the development of reports, dashboards and presentations for management and programme stakeholders

  • Build and maintain strong working relationships with internal clinical/operations teams and external community partners

  • Identify operational gaps and drive process improvements to strengthen programme efficiency, accessibility and service delivery

  • Support new community initiatives, pilots and other population health programmes as required

What we're looking for

  • Bachelor Degree in Healthcare/ Public Health/ Business/ Social Sciences or related studies

  • 3 - 5 years of working experience in community healthcare operations/ programme management/ population health

  • Hands-on experience in programme implementation, project planning and process improvement

  • Proficient in Microsoft Excel and PowerPoint, with experience in data analysis, reporting or dashboards

  • Able to track KPIs, programme outcomes and operational performance

  • Experience in budget tracking, procurement, vendor coordination or resource planning will be advantageous

  • Strong stakeholder management experience across healthcare, community care or social service partners

  • Exposure to UAT, digitalisation or system/process implementation is a plus

  • Able to manage multiple projects and deliverables in a fast-paced environment
