Care Coordinator (Cardiac Care Programme, Non-Clinical Nursing Duties)

Job Description:

• Assess and identify suitable patients for enrolment into the Transitional Shared Care Programme (TSCP), and coordinate individualized care plans based on their clinical needs.

• Coordinate patients’ follow-up appointments across cardiology clinics, primary care networks, NUP and General Practitioners, ensuring smooth transitions between specialist and primary care settings.

• Monitor patients’ treatment progress, appointment attendance, adherence to care plans and achievement of clinical targets; follow up with patients who default appointments and provide appropriate support.

• Collaborate with cardiologists, nurses, allied health professionals and primary care partners to address patients’ care needs, provide education and support treatment adherence and self-management.

• Maintain accurate patient records and programme databases, tracking enrolment, attendance, clinical progress and outcomes; support programme monitoring and evaluation.

• Support clinical audit and quality improvement activities by collecting and analysing cardiac patient data, including AMI mortality, diagnoses, procedures, medications, laboratory results and treatment outcomes, and preparing timely reports for relevant authorities and departments.
  

Requirements:

• Minimum Bachelor of Nursing or Non-registrable MBBS

• Minimum 3 years relevant experience.
