As a member of the Singapore Hospice Council (SHC) and the Asia Pacific Hospice Network (APHN), our mission is to provide dignity, comfort, and holistic support to patients and their families during life’s final journey—all within the sanctuary of their own homes.
The Role: What You’ll Do
As a Palliative Staff Nurse, you are the heartbeat of our home-care service. You will provide high-touch, person-centered nursing care, specializing in symptom management and end-of-life support. You will work independently in the community, serving as a clinical expert and an emotional anchor for families.
Key Responsibilities
1. Clinical Excellence & Symptom Management
Conduct comprehensive home-based nursing assessments for palliative patients.
Develop, implement, and review Individualized Care Plans focused on pain relief and symptom management (e.g., breathlessness, nausea, wound care).
Administer medications and perform nursing procedures (e.g., NGT, catheter care, venting) in accordance with physician orders and clinical protocols.
2. Caregiver Empowerment & Education
Train and support caregivers on proper nursing techniques, medication administration, and recognizing signs of end-of-life transitions.
Provide psychosocial support and bereavement care to families, ensuring they feel supported through the grieving process.
3. Coordination & Advocacy
Act as a liaison between the patient, family, and the multi-disciplinary team (Doctors, Medical Social Workers).
Coordinate seamless referrals to inpatient hospices, community resources, or emergency services when necessary.
4. Professional Standards
Maintain meticulous and timely clinical documentation (Target: Average of 6 home visits per day).
Ensure all care aligns with the National Guidelines for Palliative Care and SNB standards.
Participate in multidisciplinary case conferences to optimize patient outcomes.
Requirements
Qualifications: Registered Nurse with a valid Singapore Nursing Board (SNB) practicing certificate. A Diploma or Degree in Nursing is required.
Specialization: An Advanced Diploma or Graduate Certificate in Palliative Care or Gerontology is highly preferred.
Experience: Minimum 3 years of clinical nursing experience. Prior experience in Home Care, Community Nursing, or Oncology is a significant advantage.
Core Competencies: * Strong clinical judgment and the ability to work independently in a home setting.
Excellent communication skills to navigate sensitive end-of-life conversations.
