Salary: $4,500–$5,000
Working Hours: 5 Days, Office Hours

Job Scope:

Provide nursing care to patients in their homes
Conduct assessments and develop individualised care plans
Train and support patients' caregivers
Support End-of-Life care for homebound patients
Maintain documentation and coordinate with the care team

Requirements:

Valid SNB registration
Minimum 4 years of Medical / Geriatric nursing experience
Recent acute or community nursing experience required
Oncology / Palliative Care experience is an advantage
Able to support rotational after-hours tele-consult
