Job Summary

The Rehabilitation Team Lead is responsible for leading and coordinating Ju Eng Home’s rehabilitation services to ensure residents receive safe, appropriate and person-centred rehabilitation interventions that support mobility, functional independence, quality of lifeand participation in daily activities.

The role provides clinical and operational leadership to the Rehabilitation Team, oversees service delivery and caseload management, supports clinical standards and staff development, and works closely with Nursing, Medical, Social Work and other departments to ensure rehabilitation goals are integrated into residents’ overall care plans.

The Rehabilitation Team Lead is also responsible for monitoring rehabilitation outcomes, service quality, documentation standards, equipment needs and continuous improvement initiatives across the Home.

Key Responsibilities

1. Rehabilitation Service Leadership

· Lead and coordinate the day-to-day operations of the Rehabilitation Team.

· Ensure rehabilitation services are delivered safely, effectively and in accordance with residents’ assessed needs.

· Develop appropriate workflows, service standards and team practices.

· Allocate resources and manpower according to resident needs and service priorities.

· Monitor workload, caseload distribution and service capacity.

· Escalate service gaps, risks and manpower concerns to management in a timely manner.

2. Clinical Assessment & Rehabilitation Planning

· Oversee rehabilitation assessments and interventions for residents.

· Ensure appropriate rehabilitation goals and treatment plans are developed based nonresident's’ clinical and functional needs.

· Review complex or high-risk rehabilitation cases and provide guidance to team members.

· Support appropriate discharge, maintenance or transition planning for residents whose rehabilitation needs change.

· Ensure rehabilitation interventions support residents’ independence, mobility, comfort and quality of life.

3. Multidisciplinary Care

· Work closely with Nursing, Medical, Social Work, Dietetics and other relevant teams to coordinate residents’ care.

· Participatein multidisciplinary case discussions and care-plan reviews.

· Provide rehabilitation input on residents’ mobility, transfer ability, functional status, falls risk and activities of daily living.

· Ensure rehabilitation recommendations are clearly communicated to relevant care staff.

· Support appropriate caregiver or staff education where required.

4. Falls Prevention & Mobility Management

· Work with Nursing and Quality & Risk Management teams on falls-prevention initiatives.

· Assess residents with mobility deterioration or repeated falls where rehabilitation intervention is appropriate.

· Recommend suitable mobility aids, transfer techniques and rehabilitation interventions.

· Support review of falls trends and implementation of appropriate preventive measures.

· Monitor effectiveness of rehabilitation-related risk-reduction interventions.

5. Clinical Governance &Quality

· Establish and maintain appropriate rehabilitation clinical standards and practices.

· Monitor compliance with rehabilitation policies, procedures and documentation requirements.

· Conduct periodic clinical audits and quality reviews.

· Support incident reviews involving rehabilitation services where appropriate.

· Develop and monitor corrective and improvement actions arising from audits, incidents or feedback.

· Promote evidence-informed and person-centred rehabilitation practices.

6. Rehabilitation Outcomes &xPerformance Monitoring

· Develop and monitor appropriate rehabilitation service indicators.

· Track residents’ functional and rehabilitation outcomes where applicable.

· Monitor service utilisation, caseloads and intervention frequency.

· Analyse trends and identify opportunities for service improvement.

· Prepare periodic reports and updates for management.

· Key indicators may include:

· Rehabilitation caseload

· Assessment turnaround time

· Functional outcomes

· Falls-related rehabilitation interventions

· Therapy attendance

· Equipment utilisation

· Documentation compliance

· Resident and caregiver feedback

7. Team Leadership & Staff Development

· Provide supervision, coaching and guidance to rehabilitation staff.

· Support orientation and onboarding of new team members.

· Identify competency and training needs.

· Coordinate continuing professional development and in-service training.

· Conduct performance reviews and provide regular feedback.

· Promote teamwork, accountability and professional standards.

8. Therapy Assistants &Support Staff

· Ensure Therapy Assistants operate within appropriate competencies and supervision arrangements.

· Establish clear delegation and escalation processes.

· Monitor the quality and safety of delegated rehabilitation activities.

· Ensure staff receive appropriate training before undertaking assigned rehabilitation interventions.

9. Equipment & Assistive Devices

· Oversee the appropriate use and management of rehabilitation equipment and assistive devices.

· Review requirements for wheelchairs, walking aids, transfer equipment and other rehabilitation-related devices.

· Work with relevant departments on procurement, maintenance and replacement planning.

· Ensure equipment is used safely and appropriately.

· Support assessment and recommendation of suitable devices for residents.

10. Documentation & Records

· Ensure rehabilitation documentation is accurate, timely and complete.

· Monitor compliance with assessment, treatment, progress-note and care-plan requirements.

· Maintain appropriate professional and organisational records.

· Support improvements in documentation standards and electronic records where applicable.

11. Policies, SOPs & Regulatory Compliance

· Develop, review and maintain rehabilitation-related policies and SOPs.

· Ensure rehabilitation services comply with applicable regulatory, licensing and professional requirements.

· Work with Quality & Risk Management on SOP governance, audits and compliance matters.

· Support preparations for regulatory inspections and external audits.

· Ensure follow-up actions are completed within agreed timelines.

12. Service Development & Continuous Improvement

· Identify opportunities to strengthen JEH’s rehabilitation services.

· Develop programmes that support mobility, functional maintenance, frailty management and meaningful activity.

· Evaluate rehabilitation models and propose improvements based on residents’ changing needs.

· Support digitalisation, data tracking and process improvement initiatives.

· Participatein organisation-wide quality improvement projects.

13. External Service Providers

· Where rehabilitation services are outsourced or supplemented by external providers, coordinate and monitor service delivery.

· Ensure external providers comply with JEH’s service requirements, documentation standards and agreed scope.

· Monitor performance, service utilisation and issues requiring follow-up.

· Verify that service delivery and supporting documentation are appropriately maintained.

· Escalate performance or contractual concerns to management.

Requirements

· Degree or recognised professional qualification in Physiotherapy, Occupational Therapy or another relevant rehabilitation discipline.

· Full registration with the relevant Singapore professional body where required.

· Preferably at least 5 years of relevant clinical experience, with prior supervisory orteam-lead experience.

· Experience in nursing homes, community care, geriatric rehabilitation or eldercare would be highly advantageous.

· Strong knowledge of rehabilitation assessment, mobility, functional training and fallsprevention.

· Experience in clinical governance, audits and quality improvement would be advantageous.

· Strong leadership, communication and multidisciplinary collaboration skills.

· Able to manage both clinical and operational responsibilities.
